      目的 研究2022-2023年河南省居民伤害流行特征和疾病负担,为确定伤害优先防控领域提供依据。方法 采用多阶段分层整群随机抽样的方法,调查河南省14个县(区)居民39 300人2022年6月1日至2023年5月31日的伤害发生情况,根据2023年死因监测结果获得伤害死亡情况。使用伤害就医、住院及休息情况,经济负担,死亡率,潜在减寿年数(PYLL)和潜在工作损失年数(WPYLL)等评价伤害的负担。结果 2022-2023年河南省伤害发生率为6.44%,男性(6.93%)高于女性(5.94%);0~岁(6.88%)、18~岁(4.91%)和≥60岁(10.36%)人群的伤害发生率差异有统计学意义(χ2=27.99,P<0.001);非故意伤害占全部伤害类型的99.15%,前3位伤害发生原因分别为跌倒(3.38%)、道路交通伤害(1.22%)和动物伤(0.85%)。河南省伤害死亡率为38.32/10万,男性(51.25/10万)高于女性(24.67/10万),农村(38.83/10万)高于城市(36.71/10万),前3位伤害死亡原因依次为非故意道路交通伤害(14.68/10万)、意外跌倒(8.17/10万)、自杀(5.59/10万)。估算调查期间河南省因伤住院天数为2 498.56万天,因伤休息天数为15 129.89万天,伤害死亡的PYLL标化率和WPYLL标化率排前3位依次为非故意道路交通伤害、自杀、意外跌倒;道路交通伤害产生的均次经济损失最高(14 399.00元)。结论 男性、农村人群、≥60岁老年人和0~17岁儿童是河南省伤害发生和伤害死亡的重点人群。跌倒和道路交通伤害是河南省伤害发生和伤害死亡的重要类型,其中道路交通伤害造成的经济负担最高,需依据不同人群及伤害类型特点制定并实施相应的干预措施。

      Objective To study the epidemiological characteristics and disease burden of injuries among residents in Henan Province from 2022 to 2023, providing evidence for identifying priority areas in injury prevention and control. Methods A multi-stage stratified cluster random sampling method was used to investigate the injury occurrence among 39 300 residents in 14 counties (districts) of Henan Province from June 1, 2022, to May 31, 2023. Injury mortality data were obtained from the 2023 cause-of-death surveillance results. The burden of injuries was assessed using indicators including medical treatment for injuries, hospitalization and rest duration, economic burden, mortality rate, potential years of life lost (PYLL), and work years of potential life lost (WPYLL). Results The injury incidence rate in Henan Province during 2022-2023 was 6.44%, with males (6.93%) higher than females (5.94%). Significant differences (χ2=27.99, P<0.001) were observed among children aged 0-17 (6.88%), adults aged 18-59 (4.91%), and those aged ≥60 (10.36%); Unintentional injuries accounted for 99.15% of all injury types, with the top three causes being falls (3.38%), road traffic injuries (1.22%), and animal-related injuries (0.85%). The injury mortality rate in Henan Province was 38.32 per 100 000, with a higher rate among males (51.25/100 000) than among females (24.67/100 000) and in rural areas (38.83/100 000) than in urban areas (36.71/100 000). The top three causes of injury mortality were unintentional road traffic injuries (14.68/100 000), accidental falls (8.17/100 000), and suicide (5.59/100 000). During the investigation period, the estimated hospitalization days due to injuries in Henan Province totalled 24 985 600, and the total number of rest days due to injuries was 151 298 900. The standardized rates of PYLL and WPYLL for injury-related deaths ranked the top three causes as unintentional road traffic injuries, suicide, and accidental falls. Road traffic injuries caused the highest average economic cost per injury (14 399.00 yuan). Conclusions Males, rural residents, the elderly aged ≥60, and children aged 0-17 are the key populations for injury occurrence and mortality in Henan Province. Falls and road traffic injuries are the major types of injury occurrence and fatalities in Henan Province, with road traffic injuries imposing the highest economic burden. Therefore, targeted intervention measures should be developed and implemented based on the characteristics of different populations and injury types.
