对472名医院工作人员进行了一年的随访观察（1982.1~1983.6），平均每人随访了12.85个月，用固相放射免疫分析法（SPRIA）检查了他们的甲、乙型病毒性肝炎血清学标记。结果看到发生20例HBV新感染者，而没有发生HAV新感染者。HBV新感染者中以隐性感染为主，仅1例为急性HB。在HBV标记阴性者（HBV易感者）中，隐性感染与显性感染之比为8：1。其中医务人员新感染HBV的机会大大高于非医务人员（19/399对1/73；RR=3.47）。HBV新感染者的感染途径不清楚。
本文还报告了46名注射国产疫苗的结果：副作用小、安全、免疫效果好，抗-HBs阳转率在注射后11个月时为95.6%。 |

From January, 1982 to June, 1983, 613 hospital staff working in PLA 301 Hospital in Beijing suburb were observed. Forty hundred seventy and two of 613 personnel were retested for anti-HAV, HBsAg, anti-HBc and anti-HBs by solid-phase radio immunoassy (SPRIA) and SGPT. Of the 49 staff members whose anti-HAV was negative in 1982, one of them became positive in 1983. However, altogather 20 individuals were infected by HBV. Out of 223 persons without HBV markers in 1982, 9 showed seroconversion in 1983. The other 11 of the 20 persons showed four or morefold increase in titers of anti-HBs. Only one of the 20 persons had acute HB-an overt HBV infection, whereas all the rest had inapparent HBV infection without rise in the level of SGPT. The ratio of the apparent to inapparent HBV infection was 1:8 among the personnel without HBV markers. The HB incidence rate was 2.1/1000. The frequency (4.7%, 19/399) of HBV infection in the clinical workers was significantly higher than that (1.4%, 1/73) in the nonclinical workers.
Forty-six workers, all of whom HBV markers were negative in 1982 received IM injection of HB vaccine Batch No. 813 produced in Beijing Vaccine and Serum Institution, for three doses at month O, 1, 6. None became HBV infected three months after the frist injection, and 95.6 per cent of them showed an anti-HBs response 11 months after the first inoculation. The result shows that the HB vaccine produced in China is safe and immunogenic. |
