1984年9月15日北京铁路职工子弟第五小学发生一起严重的食物中毒。其原因:在自制、存放茶鸡蛋过程中被EIEC污染,该菌有其生长繁殖良机;在售出时未经加热处理,并同时产生交叉污染丝糕,使进食茶鸡蛋、丝糕者中毒发病。发病312人,发病率82.5%,病死1人,病死率0.32%。儿童发病率明显高于成人,并症状重,潜伏期短;主要表现以发烧头痛头晕恶心、腹痛、腹泻、稀便、粘液便,血、便常规为炎症表现,酷似菌痢。
在食品、病人大便中均分离到EIEC菌株。病人EIEC双份血清抗体呈四倍增长。豚鼠角膜毒力试验为阳性。
经住院抢救治疗和采取防疫措施,于9月19日发病终止,26日临床基本治愈,未见续发。 |

This report deals with an outbreak of food poisoning caused by an invasive E.coli EIEC belonging to 028ac:K73 (B):H-serotype in september 1984 in a Railway workers' Children school. The food served to the children was tea boiled eggs, soap and steamed corn cakes and was detected after the outbreak to be contaminated with EIEC strain. Thus among 271 samples taken, 31 showed EIEC while 2 of the isolates were discovered among 2 cooks just recovered from acute diarrhea.
It was estimated that 209 children had taken the meal as lunch and eventually 196 became sick, i, e, 93.8% while 54 out of 82 adults contracted the disease at the same time with milder clinical course. Moreover, there was one death with 0.32% mortality. The outbreak of illness ceased 4 days later and all patients cured 11 days after the onset of outbreak involving those hospitalized.
Consequantly preventive measures were undertaken promptly including disinfection of the canteen and patients' household and also preventive therapy to those closely contacted espicially the cooks. Meanwhile intensive surveillanceof those cooks for pathogen was achieved. |
