      目的?? 了解新生儿肺炎治疗中抗生素的使用现状和评价不同药物治疗的时间效应。 方法?? 分层抽样对湖南省5 所不同级别医院中5 年内的所有新生儿肺炎住院病历进行回顾性调查:运用Kaplan-Meier ( K-M) 法对不同的抗生素组配进行时间- 效应分析。结果?? 685 例新生儿肺炎共使用26 种抗菌药物, 其中选用了青霉素和氨苄青霉素的病例最多,分别为287 例和269 例:头孢菌素类药物亦成为新生儿肺炎的主要抗菌药物。用药以两种抗生素联合使用最多, K-M 法分析发现单用青霉素组的治愈时间短于其他方案组。结论?? 目前治疗新生儿肺炎仍以青霉素和氨苄青霉素为主, 头孢菌素类药物使用的增加和盲目的抗生素联合使用应引起重视。K-M 法可更全面的评价不同药物疗效

      Objective?? To understand the results of antibiotics use in curing neonatal pneumonia and to evaluate the time- effect of different drugs. Methods?? Throug h str atified sampling, all the hospitalized cases of neonatal pneumonia from 5 hospitals w ith different levels in Hunan province in five years were retrospectively studied. Analysis of time- effect for different antibio tics was done thr ough Kaplan-Meier.Results?? T wenty-six kinds of ant-i bacter ium drugs were used in 685 cases of neonatal eumonia, among which penicillin and ampicilin were mostly used but cephalosporins also became one of the main drugs used in treating neonatal pneumonia. Most cases were disco ver ed ( 51. 1%) using tw o kinds of antibiotics during the course of diseases through time- effect analysis. We found the effect of penicillin w as better than others in treating neonatal pneumonia w hen used as basic medicine. It was not desirable to use two or more medicines at the beginning of the tr eatment. Conclusion?? Penicilline and ampicillin were still the main drug s used in tr eating neonatal pneumo nia but mor e cephalospor ins were used than ever. The blindness in apply ing antibiotics should be recov ered. From Kaplan-Meier analysis, we could better understand and evaluate the time- effects of different drug treatments.
