Abstract:
      目的探讨中国人群肿瘤坏死因子一a(TNF.ct)基因启动子区308位点多态性与慢性HBV感染的关联性。方法检索中外数据库于2001年1月至2013年5月公开发表的文献,纳入有关TNF.a一308基因多态性与慢性HBV感染关联的病例对照研究。对入选文献提取数据后,使用Stata 12.0软件进行Meta分析。结果共纳入11项关于TNF.旺一308基因多态性与慢性HBV感染关联的病例对照研究,其中病例组1 872人,对照组1 471人。Meta分析结果表明:在全部遗传模型中均能观察到A等位基因的保护作用(Avs.G:OR=O.579,95%CI:0.422~0.794;GA粥.GG:OR=O.525,95%C1:0.325~0.847;AA口5.GG:OR=O.301,95%CI:0。125~0.720;GA+AA"s.GG:OR=0.490,95%CI:0.309-0.777;AAus.GA+GG:OR=O.542,95%CI:0.394~0.746)。进一步分层分析显示,中国北方人群中TNF.旺一308A等位基因的表达可以显著降低其慢性HBV感染的风险,而在选择自愈型患者作为对照和使用PCR作为检测手段的研究中并未发现A等位基因的保护作用。结论中国人群中TNF-a一308基因多态性与慢性HBV感染存在关联,携带A等位基因是慢性HBV感染的保护性因素。
English Abstract:
      Objective To explore the association between TNF gene 308 locus polymorphismand the risk of chronic HBV infection in Chinese population.Methods PubMed. Embase,CNKI and Sino Med database were searched for relevant articles published from January 200l to Mav 2013.Case control studies on TNF-n-308 polymorphism with chronic HBV infection in Chinese populationwere gathered with meta-analysis applied for calculation of pooled OR value(with 95%CI)afterdata abstraction.ResulIs Eleven case.eontrol studies for the TNF.ct-308 polyrnorphism with aIotal of 1 872 cases and 1 47 l controls were included.Results from the overall meta.analysis indicaredthat-308A allele had a significant decreased risk of chronic HBV infection(A.G:=0.579.95%CI:0.422-0.794;GA.GG:OR=0.525,95%CI:0,325-0.847;AA W.GG:OR-0.301.95%C/:O.125-0.720;GA+AA.GG:OR=0.490,95%CI:0.309-0.777:AA vs.GA+GG:OR=O.542.95%CI:0.394-0.746).In subgroup analyses by region.a significantlydecreased risk seemedassociated with-308A allele in population from northern China.When stratified by case type.controltype and genotyping Methods.there apppeared no significant association between TNF-0t-308A alleleand chronic HBV infection in the studies when spontaneously recovered one’s were used as controlsand PCR used as its methodology.Condusion TNF-a-308A allele seemed a protective factor forchronic HBV infection in the Chinese population.
